Welcome to Exemplar Health! We are pleased that you would like the opportunity to sell our PDP plans. Prior to marketing or selling any product, you MUST follow each step below to ensure accurate completion and efficient processing of your Broker Agreement.

Getting Contracted & Certified

Step 1: Complete Agent Appointment Packet

Please view the following Agency Service Agreement, which contains the following forms:

• Agent Contract
• Medicare Market Conduct Agreement
• HIPAA Business Associate Contract Privacy Addendum
• Consent to Background Check with Disclaimer
• Commission Schedule
• W9 (EIA agents only)

You will also need to upload the following items:

• State License
• Errors and Omissions Insurance Policy

After completing the Agent Appointment Packet the Agent will receive an email with instructions and Enrollment Code to access Exemplar University.

Exemplar Health will process the Appointment Paperwork and send the Agent a Welcome Email with the Agent Writing Number.

Step 2: Complete Medicare 101 Certification, Product Knowledge and Enrollment Training

Exemplar Health certifications are transmitted electronically for verification and processing; you are not required to fax or email certification documents. At the completion of all courses below you will be able to print the completion certificates to retain for your records.

• Go to the Training site and complete Exemplar Health Certification Training on Medicare Basics, Fraud Waste & Abuse, Product & Process, Enrollment and Compliance.

Important Note – You cannot market or sell any Exemplar Health Medicare Products until your receive a confirmation email from Exemplar Health Broker Contracting acknowledging your application submission and completion of both the Fraud Waste & Abuse and Compliance Exam and the Certification Training.

You may contract with Exemplar Health to sell PDP plans as either an Independent Broker or a Selling Agent but not both. To assist you with your application submission, you can now contract with Exemplar Health online.
